Norfolk Births, Charlotteville - In the Matter of the Registration of a Birth which has not been registered with the Division Registrar within one year after such birth took place, To Wit: And in the Matter of the Vital Statistics Act. Name of Child Horace Charles Mabee, Born February 20th 1871, Place of Birth Township of Charlotteville, County Norfolk, Address of Father at time of Birth, Forestville, Charlotteville Township, Occupation Farmer, Maiden Name of Mother Pamelia Elliott, Physician at Birth None, I, Lydia Bingleman of the Township of Charlotteville in the County of Norfolk in the Province of Ontario Do Solemnly Declare as follows:
(1) That I am the Half Sister of the aforesaid;
(2) That I have personal knowledge of the matters herein set forth, and the information herein is true in substance and in fact;
(3) (a) That both parents of the said person are dead; that I am the older half sister of such person; that I am 12 years his senior in age; that at the time of his birth, I was present in the house in the house in which he was born, and have a distinct recollection of the occurence of his birth.
And I make this solemn declaration conscientiously believing it to be true and knowing that it is of the same force and effect as if made under oath and by virtue of the CANADA EVIDENCE ACT.
